Job Summary

To support patient care initiatives, the full-time remote Care Coordinator will manage a panel of 400-450 patients, providing health coaching and conducting monthly outreach while collaborating with clinical teams to address patient needs.

Key responsibilities:

• Conduct monthly outreach and health coaching for assigned patients, ensuring continuity of care
• Collaborate with clinical teams to identify and address patients' clinical and social needs
• Document patient interactions in real time while maintaining HIPAA compliance

Required qualifications:

• Must hold a nationally recognized medical assistant certification or Certified Pharmacy Technician license
• Minimum 2 years of clinical experience in a primary care, hospital, or specialty healthcare setting
• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability
• Must be legally authorized to work in the United States
