Job Summary

Compassionate and patient-centered, the full-time remote Care Coordinator will manage a panel of 400-450 patients, conducting monthly outreach and providing health coaching to improve patient outcomes.

Key responsibilities:

• Conduct 50-60+ daily outbound calls to patients for care plan support and health coaching
• Collaborate with clinical teams to address patient needs and close gaps in care
• Document all patient interactions in real time while maintaining HIPAA compliance

Required qualifications:

• Must hold a nationally recognized medical assistant certification or Certified Pharmacy Technician license
• Minimum 2 years of clinical experience in a healthcare setting
• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
• HIPAA-compliant home office setup with minimum internet capability of 50 Mbps download / 5 Mbps upload
• Must be legally authorized to work in the United States
