Job Summary
To support integrated primary care, the full-time Patient Outreach Specialist will manage and coordinate care for patient panels, monitor patient attribution, and facilitate outreach to ensure optimal health and wellness in a remote role.
Key responsibilities
- Manage and coordinate preventative screenings and wellness strategies for patients
- Monitor patient attribution and maintain the integrity of electronic medical records
- Collect and communicate patient data to identify barriers to care and support the healthcare team
Required qualifications
- Background as a Certified Medical Assistant (CMA) is required
- Post-secondary education of one to two years in a healthcare or medical-related field is preferred
- A minimum of one year of experience in ambulatory care, community outreach, or patient support is preferred
- Training in Motivational Interviewing and/or Mental Health First Aid is preferred
