Job Summary
To support the enrollment process in a palliative care program, the full-time Certified Medical Assistant will conduct outreach calls to patients and families, complete eligibility screenings, and schedule initial visits with nurse practitioners, all while working remotely.
Key responsibilities
- Conduct targeted telephonic outreach to educate patients and families on palliative care and Vynca's services
- Complete structured screening forms to determine program eligibility based on chronic disease history and functional decline
- Act as a liaison between patients and the care team, ensuring timely responses to inquiries and coordinating logistics for patient visits
Required qualifications
- Current CMA or CCMA certificate issued by a recognized credentialing body
- 2 years of healthcare experience, with at least 8 months of clinical experience preferred
- 1 year of telephonic or call center customer service experience, with a preference for call center experience
- Experience in a medical practice or healthcare setting, including familiarity with EHR platforms
- Training in motivational interviewing and patient education, with comfort discussing serious illness and chronic disease topics
