Certified Medical Assistant
Job Summary
To support proactive care coordination for patients with chronic conditions, the fully remote Certified Medical Assistant will conduct patient outreach, manage care plans, and ensure compliance with documentation and billing standards while working within Pacific Time hours.
Key responsibilities
- Support patient enrollment and eligibility verification for chronic care management
- Conduct weekly outreach to enrolled patients, reinforcing care plans and identifying barriers to adherence
- Document patient interactions accurately and maintain compliance with billing and HIPAA standards
Required qualifications
- Certified Medical Assistant
- Bilingual (Spanish/English)
- 1-3 years of healthcare experience, preferably in chronic disease or nephrology
- Strong clinical assessment and documentation skills
- Intermediate skills with MS Office Suite and ability to work in dual systems (EHR + CCM platform)
