Certified Medical Assistant Coordinator

Job Summary

A company is looking for a Certified Medical Assistant (CMA)/Care Coordinator.

Key Responsibilities

• Conduct monthly CCM phone calls with patients and healthcare practices to ensure patient needs are met
• Create and revise personalized care plans in coordination with healthcare providers
• Identify social determinants of health and collaborate with community resources to address patient barriers to care

Required Qualifications

• At least 1 year of experience as a CMA/RMA, LPN, or RN with an active professional license
• Experience with multiple EMR/EHR systems and familiarity with Microsoft Office and Google Suite
• Ability to maintain a professional and compassionate tone on the phone
• Highly self-motivated and organized, with the ability to work independently
• Experience in case management or care coordination is a plus
