Job Summary

A company is looking for a Continuous Care Coach (Remote - Work From Home COA).

Key Responsibilities

• Manage patient caseload and communicate based on assessed needs
• Develop and implement personalized care plans in collaboration with patients and healthcare providers
• Facilitate access to care and support treatment adherence and medication management

Required Qualifications

• 2+ years of work experience in eye care preferred
• Active Certified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT/COMT) designation required
• Proficient with Electronic Medical Record (EMR) systems preferred
• Must have a dedicated remote workspace that is HIPAA-compliant
• Ability to work during normal business hours in patients' time zones
