Job Summary
A company is looking for a Continuous Care Coach (Remote - Work From Home COA).
Key Responsibilities
- Manage patient caseload and communicate based on patient needs
- Develop and implement personalized care plans in collaboration with patients and healthcare providers
- Educate patients and families on self-management and facilitate access to necessary care and resources
Required Qualifications
- 2+ years of work experience in eye care preferred
- Active Certified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT/COMT) designation required
- Proficient with Electronic Medical Record (EMR) systems preferred
- Must have a dedicated HIPAA-compliant workspace
- Ability to work during normal business hours in patients' time zones
