Job Summary

A company is looking for a Continuous Care Coach (Remote - Work From Home COA).

Key Responsibilities
  • Manage patient caseload and communicate based on patient needs
  • Develop and implement personalized care plans in collaboration with patients and healthcare providers
  • Educate patients and families on self-management and facilitate access to necessary care and resources
Required Qualifications
  • 2+ years of work experience in eye care preferred
  • Active Certified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT/COMT) designation required
  • Proficient with Electronic Medical Record (EMR) systems preferred
  • Must have a dedicated HIPAA-compliant workspace
  • Ability to work during normal business hours in patients' time zones
