Certified Ophthalmic Assistant (COA)

Job Summary

To support patients with chronic eye diseases, the full-time Continuous Care Coach will assess, coordinate, and manage care needs remotely, engaging with up to 15 patients daily to provide encouragement and support.

Key responsibilities

• Manage patient caseload and communicate frequently based on assessed needs
• Develop and implement personalized care plans in collaboration with patients, families, and healthcare providers
• Educate patients and families on self-management and facilitate access to necessary care and resources

Required qualifications

• Active Certified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT/COMT) designation
• 2+ years of work experience in eye care preferred
• Proficient with Electronic Medical Record (EMR) systems, preferred
• Ability to work effectively in a remote environment with reliable internet
• Strong consultative skills to provide guidance and support to patients
