Certified Patient Care Coordinator

Job Summary

Managing the cardiac remote monitoring program, the full-time Certified Patient Care Coordinator will review daily alerts and transmissions from cardiac devices, document findings, and communicate with patients and clinical staff in a remote setting.

Key responsibilities:

• Review and analyze remote monitoring software for patient transmission data and alerts
• Coordinate device monitoring programs and ensure appropriate communication with clinical staff and patients
• Educate patients on remote monitoring and maintain knowledge of new developments in device technology

Required qualifications:

• International Board of Heart Rhythm Examiners (IBHRE) certification preferred; Certified Rhythm Analysis Technician (CRAT) certification required prior to employment
• Strong knowledge of cardiac devices and rhythm interpretation
• Experience with remote device transmissions from major manufacturers such as Abbott and Medtronic
• Demonstrated ability to work independently and exhibit strong leadership skills
• Understanding of HIPAA regulations and commitment to patient confidentiality
