Job Summary
To support Primary Care teams, the full-time Certified Patient Outreach Medical Assistant will conduct outreach to patients recently discharged from the hospital, review discharge information, reconcile medications, and schedule follow-up appointments across multiple offices, with the potential for remote work after 90 days.
Key responsibilities
- Conduct outbound calls to patients recently discharged from the hospital
- Review discharge instructions and perform medication reconciliation
- Schedule follow-up appointments and track outreach efforts in the EMR
Required qualifications
- High school graduate or equivalent
- Completion of Medical Assistant training from an approved technical school or 1 year of relevant experience
- Current BLS Healthcare Provider Certification
- Certification or registration as a Medical Assistant from an accredited agency
- 2-3 years of experience as a Certified Medical Assistant
