Job Summary

A company is looking for a Chronic Care Management-Medical Assistant or Certified Nurse Assistant.

Key Responsibilities

Develop and maintain patient coaching relationships to support care plans
Monitor patient adherence to care plans and communicate findings to the care team
Assist new patients in the enrollment process and act as a liaison between patients and providers

Required Qualifications

High School diploma or GED
1 year of experience in remote chronic care management
Experience with cardiology patients, particularly heart failure
Medical Assistant (MA) or Certified Nurse Assistant (CNA) certification
Familiarity with EMRs, such as Athena, NexGen, GEMMS, and e-clinical works
