Chronic Disease Manager

Job Summary

A company is looking for a Manager of Chronic Disease.

Key Responsibilities:

• Supervise the chronic disease management team personnel, including hiring, training, and performance appraisal
• Establish work procedures and processes to support company standards and strategic directives
• Oversee negotiation and implementation of cost management strategies for quality outcomes

Required Qualifications:

• Undergraduate degree or equivalent experience, preferably as a registered nurse or medical social worker
• 5+ years of related experience with at least 2 years in a supervisory capacity
• 1+ years of managed care experience in Chronic Disease or Case Management
