Job Summary

A company is looking for a Lead Practice Liaison, Chronic Therapies.

Key Responsibilities

• Coordinate and communicate with Chronic providers to manage referral flow and enhance patient care
• Identify and resolve referral flow obstacles, collaborating with sales and operations teams
• Develop strong relationships with key accounts to foster improved referral processes and outcomes

Required Qualifications

• High School Diploma with 3 years of customer service/account management experience or an Associate's Degree, LPN, or Pharm Tech license
• Proven experience in building strong internal relationships with Operations/Sales Teams
• Expertise in managing the referral process and resolving referral flow obstacles
• Knowledge of the requirements for referrals in various therapeutic areas
• Proficient in using relevant platforms for referral management and data entry
