CKD Care Manager

Job Summary

A company is looking for a CKD Care Manager.

Key Responsibilities:

• Promote quality member care and ongoing customer support for renal member population
• Provide guidance, coordination, education, and care coordination functions
• Collaborate with healthcare professionals, develop interventions, and evaluate program processes

Required Qualifications:

• Registered Nurse (RN) with active unrestricted licensure in practicing state
• Associate Degree in Nursing (ADN), Bachelor of Science in Nursing (BSN) preferred
• 2 to 5 years of Nephrology Nursing and/or Case Management experience preferred
• Experience in patient education and/or case management preferred
• Knowledge of renal and diabetes disease process and current management practices
