Job Summary

Owning a panel of clients, the remote Client Manager will guide families through the care journey, ensuring high-quality experiences while balancing operational demands in a full-time capacity.

Key Responsibilities
  • Guide clients and families through onboarding and ongoing service needs while maintaining regular contact to ensure a positive experience
  • Collaborate with internal teams to manage escalations and complex care situations, ensuring reliable schedules and timely updates
  • Build and maintain strong relationships with partner agencies, translating client needs into actionable care plans
Required Qualifications
  • 3+ years of experience in senior care or patient management and customer service roles
