Job Summary
To support a growing Care Navigation clinical team, the contract hourly Clinical Administrative Support Specialist will assist with patient care coordination, data entry into Salesforce Health Cloud, and communication with treatment providers while working approximately 40 hours per week in a remote environment.
Key responsibilities
- Enter data from incoming patient care coordination requests into Salesforce Health Cloud
- Communicate with outpatient treatment providers to confirm appointment attendance and update patient lists based on daily census reports
- Collaborate with hospital discharge planners to coordinate discharge notifications and appointment details
Required qualifications
- 2 years of experience in outpatient clinical office roles, preferably in medical or behavioral health practices
- Experience with technology solutions that support clinical workflows
- Proficiency in Microsoft Word, Excel, and Outlook
- Familiarity with Salesforce Health Cloud is preferred
- Empathy and a strong commitment to reducing barriers to behavioral health and social services for underserved populations
