Clinical Care Associate

Job Summary

A company is looking for a Clinical Care Associate to provide support to Care Managers and assist members in accessing healthcare services.

Key Responsibilities

• Coordinate and schedule healthcare services as per the member's Plan of Care
• Ensure timely follow-up with members and provide telephonic assistance as needed
• Document interactions accurately and respond to member and provider inquiries professionally

Required Qualifications

• High School Diploma/GED required
• Preferred healthcare industry experience
• Minimum of one (1) year experience in home care, discharge planning, or case management
• Knowledge of managed care principles and care management standards
• Familiarity with electronic medical records and Microsoft Office applications
