Clinical Care Associate

Job Summary

Supporting field-based Care Managers, the full-time Clinical Care Associate will coordinate healthcare services, assist members with service needs, and provide telephonic support in a remote environment.

Key responsibilities

• Assist with scheduling and coordinating service needs outlined in members' Plans of Care
• Provide timely follow-up and telephonic assistance to members, ensuring effective communication and support
• Document interactions and manage member queries while maintaining compliance with organizational policies

Required qualifications

• High School Diploma/GED required
• Preferred experience in home care, discharge planning, or case management, particularly with elderly populations
• Preferred experience in the managed care industry
• Knowledge of care management standards and managed care principles
• Strong organizational skills and proficiency in Microsoft Office applications
