Clinical Care Associate

Job Summary

Supporting field-based Care Managers, the full-time Clinical Care Associate will coordinate member services, manage appointment scheduling, and serve as the primary contact for service and provider information, all while working remotely.

Key responsibilities

• Assist with scheduling and coordinating service needs identified in the member's Plan of Care
• Ensure timely member follow-up and effective communication during telephonic contact
• Document accurately and comprehensively based on organizational policies and standards of practice

Required qualifications

• High School Diploma/GED required
• Preferred healthcare industry experience
• Minimum of one (1) year experience in home care, discharge planning, or case management, preferably with the elderly frail population
• Minimum of one (1) year experience in the managed care industry
• Strong knowledge of managed care principles and concepts, including HEDIS
