Key Responsibilities

• Collaborate with care team members to provide patient care based on best practices
• Assist patients with community resources and care navigation, escalating to licensed clinicians as needed
• Facilitate care transitions and maintain updated resources for the team

Required Qualifications

• High school diploma / GED or equivalent work experience
• Must be 18 years or older
• 1+ years of experience in a clinical setting or healthcare system
• Experience with electronic medical record systems
• Intermediate knowledge of Microsoft Office Suite
