Job Summary

A company is looking for a Clinical Care Coordinator.

Key Responsibilities

• Ensure timely appointments and scheduling for members
• Assist with the implementation and facilitation of services for members
• Document care accurately and comprehensively based on organizational policies

Required Qualifications

• High School Diploma/GED required
• Minimum of one (1) year experience in home care, discharge planning, or case management
• Preferred experience in the managed care industry
• Knowledge of managed care principles and standards of practice for service coordination
• Healthcare industry experience preferred
