Clinical Care Coordinator

Job Summary

To support patients and caregivers navigating dementia-related care, the part-time Clinical Operations Care Coordinator will manage patient communications, scheduling, documentation, and cross-team communication while working remotely.

Key Responsibilities

• Prepare and maintain patient records, manage correspondence, and coordinate medical records requests
• Conduct outreach to patients and caregivers, providing empathetic support and maintaining professional communication
• Accurately document patient requests and interactions, ensuring organized case notes for team collaboration

Required Qualifications

• Minimum 3 years of experience in care coordination, medical assisting, case management, or patient navigation
• Experience working with older adults and/or patients with dementia and their caregivers
• Strong ability to take initiative and manage multiple patients and priorities
• Experience working in an outpatient medical practice or hospital setting
• Minimum of 1 year experience working in a remote environment
