Clinical Care Lead

Job Summary

A company is looking for a Clinical Care Lead.

Key Responsibilities

• Oversee and coach staff on processes, practices, and procedures while contributing to performance reviews
• Act as a subject matter expert on the Dual Special Needs Program (DSNP) and provide support to staff
• Perform telephonic care management and outreach duties, ensuring timely follow-up with members

Required Qualifications

• High School Diploma/GED required
• Minimum of two (2) years of healthcare industry experience preferred
• At least one (1) year of experience in a managed care setting, preferably with the elderly frail population
• Knowledge of care management standards and managed care principles, including HEDIS and STARS
• Strong organizational skills and proficiency in Microsoft Office applications
