Clinical Care Liaison

Job Summary

A company is looking for a Remote Clinical Care Liaison.

Key Responsibilities

• Serve as the primary point of contact for patients, families, facility partners, and providers
• Triage and prioritize urgent care requests and time-sensitive clinical communications
• Maintain accurate patient documentation within the EMR system and manage appointment scheduling

Required Qualifications

• Bachelor's degree in Healthcare Administration, Health Sciences, Public Health, or related field preferred
• 3+ years of healthcare coordination, patient services, or clinical operations experience
• Prior experience with geriatric or Medicare Advantage populations strongly preferred
• Proficiency with EMR/EHR systems and healthcare technology platforms
• Availability for approximately 5 on-call overnight and/or weekend shifts per rotation
