Job Summary

A company is looking for a Clinical Case Manager Behavioral Health DSNP.

Key Responsibilities:

• Telephonic clinical case management with Medicare population
• Utilizes Motivational Interviewing and engagement interventions
• Completes Comprehensive Assessment and Plan of care

Required Qualifications:

• 3+ years of direct clinical practice experience
• Active and unrestricted clinical behavioral health license in state of residence
• Masters Degree in Social Work or Counseling required
• Residential broadband service with internet speeds of at least 25 mbps/3mbps
• Crisis intervention skills preferred
