Clinical Intake Specialist

Job Summary

A company is looking for a Clinical Intake Specialist who will deliver care and support to patients in a virtual cardiac rehab program.

Key Responsibilities

• Serve as the initial contact for patients joining the program and provide high-quality clinical care
• Conduct health histories, evaluate patient records, and suggest appropriate care paths
• Collaborate with the care team and technology team to ensure a smooth patient experience

Required Qualifications

• Bachelor's Degree in Exercise Physiology or equivalent (Master's preferred)
• ACSM certification (preferred)
• Minimum of 1+ years relevant experience as an Exercise Physiologist in Cardiac and/or Pulmonary Rehab
• Strong anatomy and physiology skills
• Above average proficiency with technology and experience in a virtual care setting
