Job Summary
A company is looking for a Clinical Pharmacist (MTM Support).
Key Responsibilities
- Evaluate medication profiles and recommend modifications to optimize drug therapy
- Conduct telephonic comprehensive medication reviews with members
- Collaborate with a multidisciplinary team to enhance the MTM program and associated software
Required Qualifications
- Bachelor's or Doctorate of Pharmacy degree
- Minimum one year pharmacy residency training completed or equivalent experience
- 1-2 years of experience in a pharmacy or related healthcare environment
- Current, valid & unrestricted state Pharmacy license required
- May need to obtain additional Pharmacy licensure in multiple states within six months of employment
