Clinical Pharmacist - MN Licensed

Job Summary

A company is looking for a Clinical Pharmacist to provide clinical support for safe and effective medication use and access.

Key Responsibilities

• Review and assess individual member clinical cases for medication-related opportunities and recommendations
• Provide consultative medication-related advice directly to medical plan members and interdisciplinary care management team members
• Develop and provide interdisciplinary pharmacy-related education materials and participate in pharmacist engagement program enhancements

Required Qualifications

• Active, unrestricted license to practice as a Pharmacist in the employee's state of residence
• Able to obtain active, unrestricted licensure to practice as a Pharmacist in Kansas within 6 months of hire
• 3+ years of experience as a licensed Pharmacist
• 2+ years of direct patient/member interaction
• 2+ years of experience in Health Plan (Managed Care) or hospital or long-term care experience
