Job Summary

A company is looking for a Clinical Pharmacist Patient Care - Remote.

Key Responsibilities:

• Conduct telephonic medication consultations to meet/exceed production and quality metrics
• Complete pharmacotherapy assessments and develop plans to improve medication adherence
• Document clinical consultations and participate in staff development opportunities

Required Qualifications:

• Active, unrestricted pharmacist license in the state of primary residence
• 5+ years of pharmacy practice experience, including 2+ years in Clinical Pharmacy Practice
• Experience with Medicare STARs & HEDIS Measures, including Medication Therapy Management
• Proficient with Microsoft Office applications
• Ability to manage multiple projects in a rapidly changing environment
