Job Summary
To optimize patient outcomes through telephonic interventions, the full-time Clinical Pharmacist, Multi-State Licensed will execute medication management services focused on polypharmacy, adherence, and pharmacotherapy optimization while working remotely across all US time zones.
Key responsibilities:
- Counsel patients on medication adherence, address medication-related problems, and make evidence-based recommendations
- Collaborate with healthcare teams to enhance medication management workflows and patient experiences
- Participate in the design and evaluation of pilot programs aimed at improving clinical workflows and medication outcomes
Required qualifications:
- Doctor of Pharmacy (PharmD) degree
- Active, unrestricted pharmacist license in the state of practice with multi-state licensure in at least two specified states
- Minimum of 3 years of experience in value-based care, ambulatory care, or community pharmacy settings
- In-depth knowledge of pharmacotherapy and pharmacy practice
- Proficiency in telephony platforms and various Electronic Health Record (EHR) systems
