Job Summary

To optimize patient outcomes through telephonic interventions, the full-time Clinical Pharmacist, Multi-State Licensed will execute medication management services focused on polypharmacy, adherence, and pharmacotherapy optimization while working remotely across all US time zones.

Key responsibilities:
  • Counsel patients on medication adherence, address medication-related problems, and make evidence-based recommendations
  • Collaborate with healthcare teams to enhance medication management workflows and patient experiences
  • Participate in the design and evaluation of pilot programs aimed at improving clinical workflows and medication outcomes
Required qualifications:
  • Doctor of Pharmacy (PharmD) degree
  • Active, unrestricted pharmacist license in the state of practice with multi-state licensure in at least two specified states
  • Minimum of 3 years of experience in value-based care, ambulatory care, or community pharmacy settings
  • In-depth knowledge of pharmacotherapy and pharmacy practice
  • Proficiency in telephony platforms and various Electronic Health Record (EHR) systems
