Job Summary
To support innovative pharmacy care, the full-time Clinical Pharmacist - Multi-State Licensed (Bilingual - Cantonese/Mandarin) will manage medication therapy, collaborate with healthcare providers, and educate patients on chronic disease management while working remotely during Pacific or Central hours.
Key responsibilities
- Conduct comprehensive medication reviews and develop personalized care plans for patients managing chronic conditions
- Collaborate with interdisciplinary teams to enhance disease management processes and ensure medication adherence
- Drive performance on pharmacy-related clinical measures and mentor the wider team on pharmacotherapy topics
Required qualifications
- Active pharmacist licensure in good standing in at least five states, including Alabama, Florida, or Texas
- Minimum of three years of clinical experience in direct patient care
- Fluent proficiency in Cantonese and Mandarin
- Ability to work during typical clinic hours in Mountain, Pacific, or Hawaiian time zones
- Experience with collaborative pharmacy practice agreements preferred
