Job Summary
To support innovative pharmacy care programs, the full-time Clinical Pharmacist - Multi-State Licensed will manage medication therapy for members, ensuring optimal medication adherence and chronic disease education while working remotely.
Key responsibilities
- Conduct comprehensive medication reviews and develop personalized care plans for patients managing chronic conditions
- Collaborate with interdisciplinary teams to enhance disease management processes and improve clinical outcomes
- Drive performance on pharmacy-related clinical measures and mentor the pharmacy team on pharmacotherapy topics
Required qualifications
- State licensure in good standing in at least 5 states, including Alabama, Florida, Georgia, or others as required
- A minimum of 3 years of clinical experience in direct patient care
- Experience with collaborative pharmacy practice agreements preferred
- Board certification or experience in complex geriatric care, behavioral health, or lipid management is a plus
- Comfort with AI-assisted clinical tools for documentation and workflow optimization
