Job Summary
Seeking a full-time Clinical Pharmacist - Multiple States, fluent in Spanish, who will manage medication therapy for patients, collaborate with healthcare providers, and develop educational materials, all while working remotely and obtaining licensure in multiple states.
Key responsibilities
- Conduct comprehensive medication reviews and manage chronic conditions through personalized care plans
- Drive "5-Star" performance on pharmacy-related clinical measures and address therapy gaps
- Collaborate with interdisciplinary teams to enhance disease management processes and support on-call pharmacy needs
Required qualifications
- 3 or more years of clinical experience in direct patient care as a pharmacist
- State licensure in good standing in at least 5 states, including Alabama, Florida, Georgia, Indiana, Missouri, Ohio, North Carolina, South Carolina, or Texas
- Residence in the Central, Mountain, Pacific, or Hawaiian time zones or ability to support work during typical clinic hours in those time zones
- Fluent proficiency in Spanish
- Experience with collaborative pharmacy practice agreements preferred
