Job Summary

A company is looking for a Clinical Pharmacist Market Support.

Key Responsibilities:

• Conduct telephonic patient outreach and provider-facing activities to improve STARs measures
• Analyze and interpret STARs and HEDIS reporting, providing insights to plan and provider groups
• Perform annual medication reviews and develop action plans for patient medication adherence

Required Qualifications:

• Doctor of Pharmacy degree or equivalent experience
• Active, unrestricted pharmacy license in state of primary residence and Oregon
• 5+ years of Clinical Pharmacy Practice experience
• 2+ years of experience with Medicare STARs & HEDIS Measures
• 2+ years of experience in provider-facing interactions and data analysis
