Clinical Pharmacist - TN Licensed

Job Summary

A company is looking for a Clinical Pharmacist - Remote.

Key Responsibilities:

• Conduct clinical chart reviews and interpret clinical information to optimize medication regimens
• Utilize reporting tools to identify non-adherent members and present findings to clinical teams
• Provide education on disease state management and lead meetings to improve medication adherence performance

Required Qualifications:

• Doctor of Pharmacy degree or equivalent experience
• Active, unrestricted pharmacy license
• 5+ years of Pharmacy Practice experience, including 2+ years in Clinical Practice
• Experience in a clinical setting such as Managed Care, Clinic, or Hospital
• Comfortable making clinical recommendations based on clinical guidelines
