Job Summary
A company is looking for a Clinical Pharmacist Market Support for the Clinical Excellence Team, working remotely.
Key Responsibilities:
- Conduct telephonic patient outreach and provider-facing activities to improve STARs measures
- Analyze STARs and HEDIS reporting to provide insights and best practices to provider groups
- Perform annual medication reviews and develop patient-centered medication consultation plans
Required Qualifications:
- Doctor of Pharmacy degree or equivalent experience
- Active, unrestricted pharmacy license in state of primary residence and TN
- 5+ years of clinical pharmacy practice experience
- 2+ years of experience with Medicare STARs & HEDIS measures
- 2+ years of provider-facing interaction experience
