Job Summary

To improve medication adherence rates among patients, the full-time Remote Clinical Pharmacist - TX, PA, AL, TN will lead HEDIS quality initiatives, conduct medication therapy management, and analyze data to drive health outcomes.

Key responsibilities
  • Lead efforts to enhance HEDIS medication adherence and analyze related data to identify care gaps
  • Conduct medication therapy management sessions to educate patients and address adherence barriers
  • Monitor HEDIS performance metrics and provide updates to leadership on progress and recommendations
Required qualifications
  • Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
  • Active pharmacist license in one or more of the following states: TX, PA, AL, TN
  • Minimum of 2 years of experience in HEDIS-related medication adherence or quality improvement
  • Strong knowledge of HEDIS measures related to medication adherence
  • Proficiency in using EHR systems and data analysis tools
