Job Summary
To improve medication adherence rates among patients, the full-time Remote Clinical Pharmacist - TX, PA, AL, TN will lead HEDIS quality initiatives, conduct medication therapy management, and analyze data to drive health outcomes.
Key responsibilities
- Lead efforts to enhance HEDIS medication adherence and analyze related data to identify care gaps
- Conduct medication therapy management sessions to educate patients and address adherence barriers
- Monitor HEDIS performance metrics and provide updates to leadership on progress and recommendations
Required qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
- Active pharmacist license in one or more of the following states: TX, PA, AL, TN
- Minimum of 2 years of experience in HEDIS-related medication adherence or quality improvement
- Strong knowledge of HEDIS measures related to medication adherence
- Proficiency in using EHR systems and data analysis tools
