Job Summary
To enhance medication adherence among patients, the full-time Remote Clinical Pharmacist TX, PA, AL, TN will lead initiatives focused on HEDIS quality measures, conduct medication therapy management, and analyze performance metrics to improve health outcomes.
Key responsibilities
- Lead efforts to improve HEDIS medication adherence and analyze related data to implement targeted interventions
- Conduct medication therapy management sessions to address barriers to adherence and educate patients
- Monitor HEDIS performance metrics and generate reports to assess the effectiveness of interventions
Required qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
- Active pharmacist license in TX, PA, AL, or TN
- Minimum of 2 years of experience in HEDIS-related medication adherence work or quality improvement
- Strong knowledge of HEDIS measures related to medication adherence
- Proficiency in using EHR systems and data analysis tools
