Job Summary
To improve medication adherence rates among patients, the full-time remote Clinical Pharmacist - TX, PA, AL, TN will lead HEDIS quality measure initiatives, conduct medication therapy management, and analyze data to enhance overall health outcomes.
Key responsibilities
- Lead efforts to improve HEDIS medication adherence and analyze related data to identify gaps in care
- Conduct medication therapy management sessions and work with patients to resolve adherence barriers
- Monitor HEDIS performance metrics and provide updates to leadership on progress and recommendations
Required qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
- Active pharmacist license in TX, PA, AL, or TN
- Minimum of 2 years of experience in HEDIS-related medication adherence work
- Strong knowledge of HEDIS measures related to medication adherence
- Proficiency in using electronic health records (EHR) and data analysis tools
