Key Responsibilities:

Achievement of Clinical Objectives
Review participant's health exams and initiate interventions
Coordinate with healthcare team to support total healthcare management

Required Qualifications:

Current RN license appropriate for the state
High School diploma and a Diploma in Nursing, Associate's Degree, or Bachelor's Degree in nursing
Minimum of 3 years recent clinical experience
Basic knowledge of physiology and chronic disease
Able to educate and promote behavior change in a telephonic environment
