Job Summary

A company is looking for a Clinical Transformation Lead.

Key Responsibilities

• Drives initiatives to eliminate low-value medical spending and improve care quality across clinical departments
• Builds relationships with leaders to identify opportunities and manage project execution in population health
• Monitors project performance and key population health metrics, providing recommendations for improvement

Required Qualifications, Training, and Education

• Bachelor's Degree required; Master's Degree in a related field preferred
• 3 years of experience in leading clinical or operational improvement initiatives in healthcare settings
• 3 years of experience in clinical settings or clinical operations, with a preference for clinical licensure
• 3 years of experience in change management, population health, or value-based care transformation
• Intermediate computer skills and proficiency in healthcare data systems and analytics
