Job Summary

A company is looking for a Care Coordinator.

Key Responsibilities

Build rapport with patients through monthly telephonic outreach and care coordination services
Assist patients with Chronic Care Management (CCM), Behavioral Health Integration (BHI), and Remote Physiological Monitoring (RPM)
Develop and implement individualized care plans to improve patient outcomes

Required Qualifications

Certification as a Certified Medical Assistant (CMA) or Nationally Certified Pharmacy Technician (CPhT)
2+ years of experience in care coordination or clinical healthcare roles
Strong understanding of healthcare terminology and patient-centered care principles
Experience working with EMR systems
Intermediate to advanced computer skills, with the ability to navigate multiple systems
