CNA Care Coordinator

Job Summary

A company is looking for a Care Coordinator (MA/CNA).

Key Responsibilities

• Deliver continuous patient monitoring and care coordination for chronically ill patients remotely
• Assist patients enrolled in Chronic Care Management (CCM) and Remote Physiological Monitoring (RPM) programs
• Educate patients on their care plans via telephone each month

Required Qualifications

• Certified Medical Assistant (CMA), Registered Certified Medical Assistant (RCMA), Medical Assistant diploma, or Certified Nursing Assistant (CNA) with active certification
• 2+ years of healthcare experience
• 1 year of acute care experience
• Bilingual in English and Spanish (verbal and written) required
• Experience with EMR/EHR systems and Microsoft Office preferred
