Job Summary
To provide comprehensive patient care in a fully remote capacity, the full-time Colorado Licensed Advanced Practice Provider will conduct virtual patient visits, perform assessments, and collaborate on treatment plans while ensuring accurate documentation and adherence to clinical protocols.
Key responsibilities
- Conduct virtual patient assessments and develop appropriate care plans based on clinical information
- Collaborate with clinical pharmacy teams and other healthcare providers to optimize patient treatment outcomes
- Educate patients and staff, and participate in research to enhance quality patient care and best practices
Required qualifications
- Current or ability to obtain Colorado, Wyoming, and Montana state license as an Advanced Practice Registered Nurse (APRN) or Physician Assistant
- Certification as a Nurse Practitioner or current certification from a recognized certifying organization for Physician Assistants
- Eligible for State Controlled Substance License II through V and Federal DEA Certificate II through V
- BLS Certification for Healthcare Providers and other relevant certifications (e.g., ACLS, PALS)
- Applicable experience as defined by position leadership
