Job Summary

To provide comprehensive patient care in a fully remote capacity, the full-time Colorado Licensed Advanced Practice Provider will conduct virtual patient visits, perform assessments, and collaborate on treatment plans while ensuring accurate documentation and adherence to clinical protocols.

Key responsibilities
  • Conduct virtual patient assessments and develop appropriate care plans based on clinical information
  • Collaborate with clinical pharmacy teams and other healthcare providers to optimize patient treatment outcomes
  • Educate patients and staff, and participate in research to enhance quality patient care and best practices
Required qualifications
  • Current or ability to obtain Colorado, Wyoming, and Montana state license as an Advanced Practice Registered Nurse (APRN) or Physician Assistant
  • Certification as a Nurse Practitioner or current certification from a recognized certifying organization for Physician Assistants
  • Eligible for State Controlled Substance License II through V and Federal DEA Certificate II through V
  • BLS Certification for Healthcare Providers and other relevant certifications (e.g., ACLS, PALS)
  • Applicable experience as defined by position leadership
