Job Summary
A company is looking for an Ambulatory Clinical Pharmacist to provide integrated support to patients and physicians to improve the quality of patient care.
Key Responsibilities
- Provide medication management, including reconciliation, dose optimization, and adherence counseling
- Collaborate with the interdisciplinary care team to develop individualized care plans based on evidence-based guidelines
- Monitor and evaluate medication regimens to ensure clinical criteria are met and identify opportunities for drug therapy optimization
Required Qualifications, Training, and Education
- Doctor of Pharmacy degree (PharmD) from an accredited school of pharmacy
- Licensed pharmacist in Colorado
- Certification in a relevant field of practice or completion of a postgraduate residency with direct patient care experience
- At least one year of clinical services experience under a collaborative practice agreement
- Completion of required continuing education to maintain Colorado state Pharmacist license
