Job Summary

A company is looking for an Ambulatory Clinical Pharmacist to provide integrated support to patients and physicians to improve the quality of patient care.

Key Responsibilities
  • Provide medication management, including reconciliation, dose optimization, and adherence counseling
  • Collaborate with the interdisciplinary care team to develop individualized care plans based on evidence-based guidelines
  • Monitor and evaluate medication regimens to ensure clinical criteria are met and identify opportunities for drug therapy optimization
Required Qualifications, Training, and Education
  • Doctor of Pharmacy degree (PharmD) from an accredited school of pharmacy
  • Licensed pharmacist in Colorado
  • Certification in a relevant field of practice or completion of a postgraduate residency with direct patient care experience
  • At least one year of clinical services experience under a collaborative practice agreement
  • Completion of required continuing education to maintain Colorado state Pharmacist license
