Job Summary
To support a growing practice, the full-time Colorado Licensed Case Management Coordinator will manage relationships with county and independent case managers, ensuring seamless service authorizations and care plan updates in a remote setting.
Key responsibilities
- Lead coordination with county and independent case managers for service authorizations and care plan updates
- Build and maintain relationships with Colorado Case Management Agencies (CMAs) for case reviews and resolutions
- Support authorization operations for Colorado's HCBS waiver programs, ensuring compliance with policies and requirements
Required qualifications
- 5+ years of case management experience in Colorado Medicaid LTHH settings
- Proven ability to collaborate effectively with county and CMA case managers
- In-depth knowledge of Colorado's LTHH ecosystem, including HCPF policies and prior authorization operations
- Experience working with patient families and clinical teams for care continuity
- Bachelor's degree in Healthcare Administration, Business, or equivalent preferred; Colorado Medicaid certification strongly preferred
