Job Summary
Supporting PACE participants in the Aurora, Colorado area, the contract In-Home Services Coordinator will manage scheduling, service coordination, and care plan implementation in a fully remote role for a duration of three months.
Key Responsibilities
- Serve as a liaison between participants, families, caregivers, and care teams to ensure effective service delivery
- Coordinate schedules and services with community agencies and healthcare partners while maintaining compliance
- Monitor participant coverage and staffing needs, documenting changes accurately in electronic records
Required Qualifications
- High School Diploma or GED
- Minimum 2 years of related administrative experience, including scheduling and logistics coordination
- Valid state-issued driver's license and reliable personal transportation
- Minimum 1 year of experience in a healthcare setting
- Current First Aid and BLS Certification required prior to hire
