Job Summary

Supporting PACE participants in the Aurora, Colorado area, the contract In-Home Services Coordinator will manage scheduling, service coordination, and care plan implementation in a fully remote role for a duration of three months.

Key Responsibilities
  • Serve as a liaison between participants, families, caregivers, and care teams to ensure effective service delivery
  • Coordinate schedules and services with community agencies and healthcare partners while maintaining compliance
  • Monitor participant coverage and staffing needs, documenting changes accurately in electronic records
Required Qualifications
  • High School Diploma or GED
  • Minimum 2 years of related administrative experience, including scheduling and logistics coordination
  • Valid state-issued driver's license and reliable personal transportation
  • Minimum 1 year of experience in a healthcare setting
  • Current First Aid and BLS Certification required prior to hire
