Job Summary
A company is looking for a Nurse Care Manager.
Key Responsibilities
- Collaborates with healthcare teams and patients to promote positive health outcomes
- Assesses, plans, implements, coordinates, monitors, and evaluates patient care options and services
- Facilitates communication among healthcare providers and involves patients in decision-making processes
Required Qualifications
- Graduate of an accredited Registered/Professional Nursing program
- State licensure as a Registered Nurse (RN)
- Preferred: Bachelor's degree in Nursing
- Preferred: Certified Case Manager (CCM) or relevant certification
- Preferred: 2 years of nursing experience in outpatient care and 1 year of Care Management experience
