Job Summary

A company is looking for a Nurse Care Manager.

Key Responsibilities

• Collaborates with healthcare teams and patients to promote positive health outcomes
• Assesses, plans, implements, coordinates, monitors, and evaluates patient care options and services
• Facilitates communication among healthcare providers and involves patients in decision-making processes

Required Qualifications

• Graduate of an accredited Registered/Professional Nursing program
• State licensure as a Registered Nurse (RN)
• Preferred: Bachelor's degree in Nursing
• Preferred: Certified Case Manager (CCM) or relevant certification
• Preferred: 2 years of nursing experience in outpatient care and 1 year of Care Management experience
