Job Summary
A company is looking for an RN Care Manager to coordinate and manage the care of patients with chronic or complex conditions.
Key Responsibilities
- Assess patient needs and develop individualized care plans according to NCQA standards
- Establish collaborative partnerships with patients to evaluate healthcare needs and resource utilization
- Facilitate communication among healthcare team members and monitor patient care across the continuum
Required Qualifications
- Current Registered Nurse (RN) license in state of practice
- Bachelor of Science in Nursing (BSN) from an accredited institution
- Demonstrated clinical nursing experience and familiarity with patient-related terminology
- Experience in care management
